Enrollment Information

Thank you for your interest in enrolling you child at Care For All Ages!

As part of the enrollment process, there are several forms required by the State of Wisconsin, you can download these here and fill them out in advance to save time during the enrollment process if you would like.

• Child Care Enrollment (DHFS Form F-CFS0062)
• Day Care Immunization Record (DHFS Form F-44192)
• Child Health Report - Child Care Centers (DHFS Form F-CFS0060-E)
• Health History and Emergency Care Plan (DCF Form DCF-F (CFS-2348)
• Alternate Arrival / Release Agreement – Child Care Centers (DHFS Form F-CFS0104-E)
